Incident Reporting

 

Incident reporting is intended to determine causative or contributing factors of occupational injuries and illnesses and for use in determining if any action is necessary to prevent recurrence.

Incidents to Report: 

  • Near misses 
  • Injuries
  • Illnesses
  • Exposures
  • Fatalities
  • Hazardous material spills

Faculty, Staff and Students should report any of the above incidents. There are slightly different procedures for Staff and Faculty vs. Students so please refer to the instructions for your function:

Serious Injuries or Fatalities

Supervisors and/or Campus Safety contact EHS as soon as possible to report work related death, serious injury, or illness. EHS immediately (within 8 hours) reports any serious injury, illness, or fatality to Cal/OSHA as required by Cal/OSHA (8 CCR 342).

Cal/OSHA defines an injury or illness as “serious” if it: 

  • Requires inpatient hospitalization for a period in excess of 24 hours for other than medical observation
  • An employee suffers a loss of any member of the body
  • An employee suffers any serious degree of permanent disfigurement

There are no reprisals or other job discrimination for expressing any concern, comment, suggestion or complaint about a health and safety related matter.
